Keeping in Touch

This initiative was designed to establish communication and provide support for our existing Link Friends who may be at risk of escalating needs due to isolation or other factors such as memory loss, increased care responsibilities, long-term health conditions and deteriorating physical health.

Proactive support for our community

This program takes various forms across the borough and was designed and funded by Wokingham Borough Council. For us at Link Visiting, it involves supporting those in matched friendships. We maintain close communication with our volunteers and our support involves regular phone calls and visits to our Link Friends. We also refer or ‘signpost’ to other organisations to help to reduce, delay and prevent the need for statutory services. We aim to prevent a crisis from developing by swift intervention finding solutions to problems and avoiding or reducing the risk of potential issues arising in the future. A positive outcome for everyone.

This project fits within our goal at Link Visiting: to ensure our volunteers feel supported within their role visiting vulnerable adults, and to ensure our Link Friends can continue to live independently at home for as long as possible with improved well-being and support.

A prestigious nomination

It was a fantastic achievement to be nominated for a prestigious MJ Award for Innovation in Partnerships for the Keeping In Touch (KIT) project, funded and designed by Wokingham Borough Council working alongside Age UK Berkshire and Berkshire Healthcare NHS Foundation Trust.
